Methamphetamine is a central nervous system stimulant that is abused recreationally. It can also be prescribed as a stimulant or appetite suppressant. Methamphetamine exists in two different forms: levo-methamphetamine (l-methamphetamine) and dextro-methamphetamine (d-methamphetamine). Each has the same chemical formula, but a slight variation in chemical structure produces different properties for each isomer.

Isomer d-methamphetamine is more stimulating and psychoactive while l-methamphetamine is less central nervous system -active, vasoconstrictive and is available over-the-counter as the active ingredient of the Vick’s inhaler as well. This leads to a higher abuse potential for d-methamphetamine and lower abuse potential for l-methamphetamine. The D/L isomer test will narrow down the potential sources of a methamphetamine positive result.

Drugs likely responsible for 95-100% d-methamphetamine results:

  • Illicit methamphetamine
  • Methamphetamine (Desoxyn®)
  • Benzphetamine (Didrex®)

Drugs likely responsible for 0-5% d-methamphetamine or 95-100% l-methamphetamine results:

  • Selegiline (Eldepryl®, EMSAM®, Zelapar®)
  • Over-the-counter vapor inhaler containing levmetamfetamine (Vick’s Inhaler®)
  • Illicit methamphetamine (uncommon)

Drugs NOT responsible for any positive methamphetamine results:

  • Pseudophedrine (Sudafed®) products
  • Amphetamine/ Dextroamphetamine products (Adderall®, Dexedrine®, Vyvanse®)
  • Phentermine (Adipex-P®, Lomaira®, Qsymia®)
  • Methylphenidate products (Concerta®, Ritalin®)
  • Other OTC or prescription products
